Multiple sclerosis MS ; is a progressive disease associated with damage to the central nervous system brain and spinal cord ; . A gradual deterioration in lower limb motor function and balance is seen in most patients, with varying degrees of immobility. The majority of patients with MS will also experience Lower Urinary Tract Symptoms LUTS ; as the disease progresses. The impact of symptoms such as urge urinary incontinence is compounded by immobility, and the authors emphasise that LUTS are a cause of "significant disability and frustration" in these patients. Continence advisors and urology nurse specialists are therefore important figures in the patient's support network, because urinary symptoms are often managed conservatively. However initial assessment by a urologist is necessary to determine appropriate management and to identify patients at risk of renal failure. In this regard, the incidence of renal complications has probably been underestimated, as community-based, long-term follow-up studies have not been carried out. This review article, written jointly by leading authorities in neuro-urology and MS neurology, is the most recent 1999 ; and most comprehensive 182 references ; available. Since this review was written, there have been several pharmacological advances. The availability of slow-release preparations of anticholinergic medications should have an impact on patient compliance, as side effects such as dry mouth are less common. Resiniferatoxin is still being evaluated in clinical trials whilst other treatments, such as cystoscopic Botulinum toxin detrusor injections and sublingual cannabis extract have shown promising results in preliminary studies. The NPSA has issued a new guide to help primary care organisations in England and Wales make patient care safer. This document provides a comprehensive overview of the latest thinking and evidence about patient safety within primary care, and lists actions that primary care organisations, staff and teams can take to improve patient safety locally and help meet their clinical governance targets. The most common error reported in primary care is a failure or delay in diagnosis. OVERDOSAGE Pioglitazone hydrochloride During controlled clinical trials, one case of overdose with pioglitazone was reported.A male patient took 120 mg per day for four days, then 180 mg per day for seven days. The patient denied any clinical symptoms during this period. In the event of overdosage, appropriate supportive treatment should be initiated according to patient's clinical signs and symptoms. Metformin hydrochloride Overdose of metformin hydrochloride has occurred, including ingestion of amounts 50 grams. Hypoglycemia was reported in ~10% of cases, but no causal association with metformin hydrochloride has been established.
